Acute myocarditis mimicking acute ST-elevation myocardial infarction

Abstract

A 27-year-old woman visited our hospital with the symptoms of fever, chilling, poor oral intake and aggravated dyspnea (NYHA Fc III). The chest X-ray showed cardiomegaly with both pleural effusion. Initial electrocardiogram (ECG) showed wide QRS tachycardia with precordial ST-elevation (V1-V4). Cardiac enzyme levels were elevated. The patient was delivered to coronary cath. room under the impression of ST-elevation myocardial infarction. The coronary angiographic findings were normal. The echocardiographic findings showed severe global hypokinesia with severe left ventricular (LV) dysfunction (ejection fraction=17%) and the LV internal dimension was enlarged (LVIDs/d: 57/67 mm). With the further history taking and serial ECG follow up, the patient was diagnosed finally as acute myocarditis. The extracorporeal membrane oxygenation was applied for cardiopulmonary support. Heart transplantation was performed after 1 month.
